A form of prayer, to be used upon the fifteenth of January in all churches and chappels within the cities of London and Westminster, the suburbs of each, and the borrough of Southwark : and upon the two and twentieth of the said moneth, in all other churches and chappels within the rest of His Majesties dominions of England and Wales, and the town of Berwick upon Twede : being the several days appointed for a general fast, to be kept in the respective places, for the averting those sicknesses and diseases, that dearth and scarcity, which justly may be feared from the unseasonableness of the weather.
