We examine the causes of limited action to address gay and bisexual men's and transgender women's vulnerability and the geo-political dynamics shaping their vulnerability and neglect. Specifically, we show how stigma, discrimination, and violence targeting gay and bisexual men and transgender women have served as barriers to HIV prevention and care. We explore how formal and de facto efforts to prevent sexual and gender minority community-led organizations from full participation in civil society in Africa and the Caribbean, coupled with the dynamics of development funding, constrain the ability of sexual and gender minority communities to tear down structural barriers that prevent them from accessing affirming HIV prevention and care.
