Medical therapies and non-pharmacologic measures for HF can reduce patients' need for re-hospitalization, lower costs of care, and improve survival. Unfortunately, these strategies are underutilized in conventional practice settings. Dr. Fonarow addresses multidisciplinary pathways to implement evidence-based HF therapies that optimize the care of patients with HF, including measures to prevent HF from developing in patients at risk.
