| Summary: | This short case considers a near collision between two passenger aircraft which occurred at the Fort Lauderdale-Hollywood Airport in 2007 when a taxiing United Airlines flight missed its intended turn onto a taxiway and instead entered the runway. While there is a compelling argument for the pilots to bear the blame for this event, the case challenges students to consider the role that process variation played in this and similar incidents. Using runway safety data, the case illustrates the concepts of common and special cause variation, root cause analysis, and the use of countermeasures as process improvement tools. Students are asked to analyse control charts, create fishbone diagrams, and recommend specific countermeasures that might prevent future incidents. The case can also be used to illustrate fundamental elements of Deming's quality philosophy regarding variation and process performance. |
