Description
Summary:A United States Forestry Service (USFS) plane carrying forestry workers crashed in the Rocky Mountains, killing three and seriously injuring two. When rescue agencies were unable to locate the crash, the two survivors defied the odds in a treacherous hike to safety. Subsequent investigation revealed that the pilot had been hired three months previously and had only 14 hours' experience taking off or landing in the backcountry despite USFS minimum requirements of 200 hours' flight time in "typical terrain." The Federal Aviation Administration (FAA) cited this accident in testimony to Congress as evidence for increased vigilance when selecting and training pilots for backcountry and mountain flying. The case asks students to consider how management decision making contributed to the escalation of risk. Further, students are asked to identify underlying conditions that pose risks in organizations with which they are familiar. Serious accidents can have origins in humble beginnings.
