Summary:The development of a novel measurement tool for use in health care settings can be challenging. First, it must be proven that there is a need for a new tool and that an existing one would not suffice for the purpose in question. Then the creation of the content of the new tool must be informed by existing evidence, expert experience, and knowledge from the context in which it will be used. These three elements must be blended together to create a draft measurement tool. This draft tool must then undergo a number of rounds of critique and testing by methods experts and representatives of the group on whom the tool is eventually intended for use. Finally, to prove the new tool is reliable and accurate, statistical testing must be carried out after the tool has been used with a defined number of participants. Here we will describe the challenges of this process using the development of a tool for the measurement of social exclusion in health care settings for a PhD as a worked example. As this measurement tool was developed to measure social exclusion, it needed to be field tested with the participation of a large number of people who were presumed to be socially excluded. To do that, we needed to engage with many gatekeeper services and nongovernmental organizations, and the experience of developing relationships with these gatekeepers is described in a sister case to this one.
