| Summary: | The pharmaceutical care system in the Netherlands is expected to transition from a product-oriented care toward a more patient-centered care (i.e., "good integrated pharmaceutical care"). Monitoring the changes in the system and measuring the outcomes of this transition is paramount. Therefore, back in 2014, we initiated a project aimed at developing a model on "good integrated pharmaceutical care" and selecting indicators to monitor changes in the system. Based on a literature review and interviews to experts, we came up with a definition of "good pharmaceutical care." The identified elements of "good pharmaceutical care" were then organized in a model. The multidisciplinary project team was the biggest challenge and became biggest asset of this project. On one hand, we were invested in learning each other's (scientific and professional) language; on the other hand, it enabled creative solutions, in particular the idea of building up a logic model. A strength of this project is that we used feedback from experts in the field to fine-tune the proposed model, and then explored its potential in practice by conducting two case studies with professionals as well as patients. Although the proposed logic model provided an adequate representation of what "good pharmaceutical care" in the Netherlands could be, its implementation was not straightforward per se. Special attention would be needed to consider everyone's vested interests and achieve stakeholder participation. |
