Summary:This case study derives from a research project that I led at the U.S. Department of Health and Human Services' Biomedical Advanced Research and Development Authority in 2016. The objective of this case study is to elucidate the methodological challenges in evidence synthesis and framing research question and evaluation of rate of return on investment on a public medical health by considering both direct and spillover effects. Estimating the economic spillover effect, which is the difference between the social and private return on investment in a project, can help in prioritizing research and development and a better assessment of health technology assessment in public policy investments. One of the main challenges with prioritizing and assessing different public health projects is identifying the correct and accurate key parameters. This case study demonstrates how to formulate the research questions by identifying the targeted population, intervention, comparison, outcome elements. It also shows how to synthesize the key information in estimation of the return on investment in research and development to help policy makers in making better evaluations of the health technology assessment in public health projects. This case study shows how to avoid the usual mistakes in identifying the correct key population, intervention, comparison, outcome elements, choosing the best alternative information in case of a lack of primary key parameters as well as projecting the correct return on investment and health technology assessment in public medical investment by estimating the correct market size and market share.
