Summary:In the research described in this case study, qualitative, semi-structured interviews were used to determine the reasons why clients in the Irish Methadone Maintenance Treatment Programme (MMTP) fail to stabilize appropriately and have their care transferred from the high-risk clinics to the lower risk setting of the general practitioner in the community. Semi-structured interviews allowed us to learn, from the clients' perspective, why the current system fails them. In the study, we interviewed 17 clients of one of Ireland's Health Service Executive Drug and Alcohol Services. Each client had a severe "opioid use disorder," and clients had spent on average 7.5 years engaging with the MMTP. Semi-structured interviewing revealed factors contributing to clients' sustained drug misuse as well as the positive factors influencing periods of abstinence and clients' own suggestions for improving their journeys. This case study provides an overview of the semi-structured interview process and addresses some of the challenges of, and ethical considerations with, using the method with this population. In particular, it addresses the lead researcher's dual-role as both the interviewees' clinician and a researcher.
