A comparison of two methods of femoral head and neck excision in the dog /

Abstract:ABSTRACT: This study compared standard femoral head and neck excision to femoral head and neck excision with an interposed biceps muscle flap. Sixteen 20 to 30 kg dogs had normal orthopedic examinations. Eight dogs had standard right femoral head and neck excision, and eight dogs had right femoral head and neck excision with interposition of a biceps muscle flap. Postoperative radiographs demonstrated adequate femoral head and neck excision in all dogs. Lameness was graded by the author and 3 unbiased examiners, and forceplate analysis was performed preoperatively and at 1, 2, 4, 6, and 16 weeks postoperatively. Other parameters evaluated at these times included leg length, goniometric measurements of the hip, stifle and hock, and thigh circumference. Soft tissue interposition between the ostectomy site and acetabulum was evaluated upon postmortem exam 16 weeks postoperatively. Lameness lessened in severity at week 6, but there was no difference between groups at any postoperative evaluation, and neither group returned to a completely normal gait. Upon forceplate analysis all dogs demonstrated decreased weight bearing on the operated pelvic limb. Weight bearing forces of the operated limb were increased at 6 and 16 weeks, but did not return to the preoperative value in either group. The muscle flap group demonstrated less weight bearing force on the operated limb at weeks 6 and 16 than the group with standard femoral head and neck excision. Both groups showed shortening of the operated limb, but more shortening was noted in the muscle flap group. Goniometric measurements demonstrated limited hip extension in both groups. Both groups had decreased hip, stifle, and hock angles measured in standing position. Thigh circumference measurements were decreased in the operated limb of both groups, but moreso in the muscle flap group at week 6. Postmortem measurements demonstrated more soft tissue interposition in the muscle flap group compared to the group with standard femoral head and neck excision, but the difference was not statistically significant. Skeletal muscle fibers were present in the interposed tissue of the muscle flap group, and the healed surfaces were smoother than those of the standard femoral head and neck excision group.
