In part 2, "The history of episcopacy" parts 1 and 2 each have separate dated title page, pagination, and register. The "History" is a reissue, with cancel title pages and foreword, of the 1642 edition (Wing H1722-3), of which at least part 2 was printed by Richard Hodgkinson. The last leaf is blank.
