Same typesetting, with addition of quire E, as the version with imprint: London printed for Henrie Tomes. An. 1604 (STC 1118). Some copies have E outer forme printed and the text completed in MS.--STC Copy at reel 1505:7 identified in reel guide as STC 1119. Imperfect: copy at 1980:10 torn and stained, pp. 30-31, 34-35, 37-40 replaced by manuscript surrogates in two different hands. Reproductions of originals in the Library of Congress (Washington, D.C.)
