Summary:The surgical treatment of epilepsy is an expanding area of endeavour and an expertise that remains underutilised in many countries. This book includes the contributions that were discussed and modified accordingly at the Fourth Advanced Seminar in Neurosurgical Research held on May 17th-18th, 1989 in Padua, Italy. It presents current reviews on: the epileptic focus, the pathological focus, membrane electrophysiology of epileptiform activity, the pathophysiology of acute brain damage, neuropathology, the epidemiology of drug resistant epilepsy, the adverse effects of anti-epileptic drugs and post-marketing surveillance, controversies in post-traumatic epilepsy and epilepsy following neurosurgical intervention, the role of prophylactic therapy. The principles of surgical treatment for epilepsy are discussed and selection criteria are given, including electrophysiology, psychometry, neuroradiology, nuclear medicine, and the long-term results of the various surgical techniques. The discussion asks why so few patients are operated on for epilepsy despite cogent arguments in its favour, particularly when considered in the context of cost-benefit analysis and rehabilitation. The book provides ready access to a wide range of literature not available elsewhere, and the editor's wish is that further research into intractable epilepsy will be facilitated.
