This study examined if a change in a part of a kinetic chain can effect the other parts of the kinetic chain in a way that would suggest the possibility of subsequent damage. In these experiment designs, the effects of changes in the foot on the knee joint were investigated. Wedged insoles were used with healthy subjects to simulate excessive inversion and eversion, and patients with hallux rigidus and hallux valgus were studied. Spatial/temporal, kinematic and kinetic data were measured for each condition. A mathematical model was created to calculate internal joint forces. It was found that wedged insoles caused changes in the kinematics at the ankle joint. Lateral wedged insoles shifted the joint force from the medial to lateral compartment of the knee joint. Medial wedged insoles did not have any effect on the kinetics at the knee. Hallux rigidus changed the kinematics at the ankle joint only, with no effect in the kinetics at the knee joint. Hallux valgus caused changes in the kinematics at the ankle and hip joint. It was found that the contralateral knee had greater joint reaction force than the hallux valgus knee. This study showed that a change in one part of the kinetic chain could cause changes to other parts of the kinetic chain, and these changes might cause secondary injury.
