|a In the United States alone, over 100 fatalities occur annually from trench cave-ins. Although technical knowledge is available to handle this problem, the majority of cave-ins have resulted from the Lack of application of this knowledge. To date, engineers have eluded design responsibility by passing it on to contractors through exculpatory contract and specification clauses. This has resulted in Federal, state and local safety codes, as exemplified by the excavation, trenching and shoring subsection of the Occupational Safety and Health Administration (OSHA) Code. Newspapers, state agencies, insurance companies, contractors and OSHA offices were contacted to determine causes and occurrances of cave-ins. The results of several surveys are presented herein. It is concluded that the vast majority of fatality cave-ins occur in shallow excavations, primarily for sewer lines. The advent of OSHA has not significantly reduced the fatality rate. No part of the country is immune to cave-ins and every type of soil is susceptible. The design of any trench or excavation is an engineering problem much more complicated than the structural design of permanent facilities and involves soil mechanics, geology, site exploration, soil testing, experience and, above all, judgment. It is concluded that a manual of practice for the design of shallow excavations is necessary since many excavations are specified by engineers not familiar with recent geotechnical engineering advancements. The recommendations for evaluation and design procedures, which should be included in a manual, are developed in the areas of site evaluation; analysis of stability, earth pressure, settlement and bottom heave; loads on support systems; observations during and after construction; external factors Influencing trench stability; and special soil problems which need to be evaluated. These include the use of the limit analysis method for evaluating trench wall stability and the need for support, and apparent lateral earth pressure diagrams for evaluating the earth pressures exerted on a support system and the loads carried by the struts.
